Job Description
The Admissions Patient Care Manager (RN) opens the door to compassionate hospice care for patients and families beginning their end-of-life journey. This role oversees referral intake and admissions, ensuring timely, seamless support aligned with regulatory standards and organizational processes.

Requirements
- Current RN license required; BSN preferred
- Minimum five years of management experience
- Admissions and/or intake experience required; hospice/palliative care experience preferred
- Strong organizational, multitasking, and interpersonal skills
- Proficiency with Excel, Word, PowerPoint, and applicable software
- Valid Texas driver's license, reliable vehicle, and liability insurance
- Knowledge of Federal/State laws governing home health and hospice licensure
