Job Description

The Admissions Patient Care Manager (RN) opens the door to compassionate hospice care for patients and families beginning their end-of-life journey. This role oversees referral intake and admissions, ensuring timely, seamless support aligned with regulatory standards and organizational processes.

Requirements

  • Strong organizational, multitasking, and interpersonal skills
  • Proficiency with Excel, Word, PowerPoint, and applicable software
  • Valid Texas driver's license, reliable vehicle, and liability insurance
  • Knowledge of Federal/State laws governing home health and hospice licensure
