Job Description

Trinity Hospice seeks a Full Time RN Case Manager to coordinate compassionate care for patients in the SE Houston area. You'll build meaningful relationships with patients and families while leading an interdisciplinary team and maintaining autonomy in your daily practice.

What You'll Do

  • Visit patients in their homes, focusing on comfort, dignity, and quality of life
  • Assess patient needs, manage symptoms, and adjust plans of care as conditions change
  • Serve as primary nurse coordinating care for assigned patients and develop individualized care plans
  • Coordinate with physicians, hospice aides, LPNs, social workers, and chaplains
  • Support and educate families while advocating for patients and maintaining documentation in EMR

Requirements

  • Active RN license in Texas with ADN or BSN from accredited nursing program
  • 2 years of RN experience working for a hospice agency required
  • Strong clinical judgment, compassion, communication skills, and reliable transportation
  • Valid driver's license, auto insurance, and current CPR certification
  • Must live in SE Houston area to be considered

Depending on the facility, you may also handle admissions paperwork, manage wound care, supervise medication techs, and respond to emergencies.
