Job Description
Adaptive Home Health is seeking a Home Health Aide (HHA/CNA) to provide compassionate personal care to patients in their homes. This full-time, field-based role offers competitive pay, schedule flexibility, and the opportunity to work with cutting-edge technology designed to reduce administrative burden.
What You'll Do- Provide personal care assistance including bathing, dressing, grooming, and toileting
- Assist with safe transfers, ambulation, and mobility using proper body mechanics
- Monitor and report changes in patient condition and vital signs to supervising nurse
- Document care provided accurately in Homecare Homebase and communicate with care team
- Perform light housekeeping tasks and assist with range of motion exercises as ordered
- Current Texas HHA certification and CPR/BLS certification
- 1+ years of experience providing personal care in home health or clinical setting
- Current Texas driver's license and reliable transportation
- Ability to lift, transfer, and provide hands-on care safely
- Strong communication skills and high school diploma or GED
- $17.00–$18.00 per hour plus mileage reimbursement

What to Expect in This Role Day-to-Day

Throughout the shift, you will assist with meals, escort residents to activities, monitor for changes in behavior or health, and document everything in the care log. Vital signs checks happen at regular intervals, and you will be the first person to notice if something seems off with a resident.
The team dynamic is close-knit. You will work alongside other aides, report to the charge nurse, and communicate with families who visit. In memory care units, redirection and engagement activities are a big part of the role — keeping residents calm, oriented, and socially active. The work is physically and emotionally demanding, but many aides describe deep bonds with the residents they care for and a sense of purpose that keeps them in the field long-term.
