At Your Side Home Care Services- Willowbrook - PRN - Home Health RN - Registered Nurse Pay: $40.00 - $50.00 per hour Expected hours: 10 hours per week Job description: Job Summary We are seeking a compassionate and experienced Home Health Care Registered Nurse (RN) to join our dynamic team. The ideal candidate will have expertise in utilizing the OASIS system, maintaining meticulous patient records, and delivering skilled nursing care in patients' homes. This role requires proficiency in wound care management, infusion therapy, and a comprehensive understanding of home health care protocols. Key Responsibilities Conduct thorough initial and ongoing patient assessments using the OASIS tool to develop individualized care plans. Provide skilled nursing care, including wound care, infusion therapy, catheter management, and other specialized services as prescribed. Educate patients and families on managing health conditions, medications, and preventative care. Maintain accurate, detailed, and timely documentation of patient conditions, interventions, and progress in compliance with federal, state, and agency guidelines. Submit OASIS documentation promptly to ensure compliance with Medicare and agency standards. Communicate changes in patient status to the care team and implement appropriate interventions. Serve as a liaison between the patient, their family, and the care team to address questions and concerns. Adhere to best practices and evidence-based protocols to ensure the highest standard of care. Strong communication skills and the ability to build rapport with patients and families are essential. Must possess a valid nursing license and be committed to ongoing professional development. Applicants must have a minimum of 1 year of home health care nursing experience Applicants must have experience administering medication via PEG Tube (percutaneous endoscopic gastrostomy) Education & Licensure Current and unencumbered RN license in Texas Bachelor’s degree in nursing (BSN) preferred but not required. Experience Proficiency in OASIS documentation is required. Experience in wound care and infusion therapy is essential.
