Responsibilities include: Performing comprehensive nursing assessments in patients' homes. Developing, implementing, and updating individualized plans of care. Providing skilled nursing care, treatments, and patient education. Collaborating with physicians, therapists, and interdisciplinary team members to coordinate care.
