POSITION SUMMARY Assist with planning, organizing and developing the overall emotional and social needs of the residents are met/maintained on an individual basis. Provides general nursing care to residents in the retirement community and health center by performing the following duties listed below. ESSENTIAL DUTIES AND RESPONSIBILITIES Connect residents and families to necessary resources and supports in the community. Encourage residents to participate in healthy activities, example; Edgemere Masterpiece Living. Perform Resident Psycho social assessments when needed Assist /Educate residents and families to participate in decision making when there is a change in health condition. Help residents/families adjust to admission to Plaza Health care services; possible role changes; exploring emotional/social responses to illness and treatment of a resident. Educate residents and families on the different levels of health care available; entitlements, community resources and advanced directives. When needed perform health screening and evaluation of new and current residents. Follow up with incident reports, explore possible ways of preventing future such incidents. Work closely with Home Health Providers and assisting residents with coordination of additional services within their apartments. Enforce the Caregiver Handbook. Ensure residents are receiving Quality care from caregivers, Follow up with complaints. Monitor change of condition in residents and make recommendations on proper level of care.
