Full time-Onsite, Houston, Texas Position Summary: The Hospice MSW provides compassionate psychosocial support to patients and families facing end‑of‑life needs. This role delivers clinical social work services, facilitates care planning, ensures regulatory compliance, and collaborates with the interdisciplinary team to holistically support quality of life and patient/family goals. Responsibilities: • Conduct psychosocial assessments for hospice patients and caregivers. • Develop individualized care plans addressing emotional, social, and resource‑related needs. • Provide counseling, anticipatory grief support, and crisis intervention. • Assist families with community resources, long‑term care placement, and advance care planning. • Participate in IDT meetings and contribute to ongoing care coordination. • Ensure compliance with Medicare, state hospice regulations, and organizational standards. • Document timely and accurately according to regulatory and agency requirements. • Support bereavement program activities as assigned. • Educate patients/families on hospice philosophy and available supportive services. Qualifications : • Master’s degree in Social Work (MSW) required. • Current state licensure (LMSW/LCSW) according to state regulations. • Minimum 1 year of social work experience in hospice, home health, palliative care, or related setting preferred. • Clinical assessment skills and proficiency in crisis management. • Strong communication, organization, and teamwork abilities. • Valid driver’s license and reliable transportation.
