Job Description
Houston Methodist is seeking a Nurse Practitioner to serve as a Post Liver Coordinator at our Medical Center. This role combines clinical care, leadership, and quality improvement within our academic medical center environment.
What You'll Do- Coordinate patient care for defined populations under physician supervision, focusing on optimal outcomes
- Conduct health assessments, develop care plans, and evaluate patient outcomes across inpatient and outpatient settings
- Lead interprofessional rounds and collaborate with healthcare team to manage patient care
- Serve as clinical resource, mentor to staff, and preceptor for NP students and medical residents
- Drive quality improvement initiatives and implement safety protocols aligned with organizational goals
- Master's degree or higher in Nursing from accredited program with NP specialization
- Two years RN, advanced practice, or NP clinical experience
- Texas RN license, ANCC certification, BLS, ACLS, Prescriptive Authority, and DEA license
- Strong clinical assessment, communication, leadership, and independent clinical judgment skills
- Ability to work autonomously and manage multiple priorities effectively
- Position with Houston Methodist Specialty Physician Group, part of leading academic medical center
- On-call requirements and travel within Houston Metropolitan area as needed
