Job Description

Join Our Team as a Full-Time Occupational Therapist (OT) We are seeking a dedicated and compassionate Occupational Therapist (OT) to join our in-house team of outstanding professionals. In this role, you will provide person-centered care focused on improving residents’ independence, safety, and quality of life through meaningful daily activities and functional outcomes. Your Impact as an Occupational Therapist (OT) Consult with patients to evaluate their physical condition and its impact on daily occupations, well-being, and quality of life Assess and interpret patient evaluations and test results Develop individualized treatment plans using a variety of therapeutic techniques to maximize performance in activities of daily living (ADLs) and instrumental activities of daily living (IADLs) Assess patients’ home and living environments and recommend modifications based on their needs and capabilities Recommend adaptive equipment and provide training to support independence and safety Ensure all treatments are delivered in accordance with the established plan of care Monitor patient response to treatment and modify interventions as clinically appropriate in collaboration with the interdisciplinary team Provide clinical education and support to patients, family members, and caregivers Collaborate with interdisciplinary team members to ensure high-quality patient care Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations Comply with all applicable rules, regulations, and procedures Attend required meetings as designated by the Director of Rehab What Makes You a Great Fit Active and valid Texas Occupational Therapy license In good standing with all regulatory agencies and licensing boards Skilled Nursing Facility (SNF) or long-term care experience preferred Current knowledge of occupational therapy treatment practices Experience with Net Health Electronic Documentation System preferred Ability to manage patients with varying personalities and needs Strong communication, collaboration, and clinical decision-making skills Benefits (Full-Time) Health, Dental, and Vision Insurance Ancillary coverage including AD&D, Short-Term Disability, Cardiac, Cancer, Critical Illness, and Hospital Confinement Whole and Term Life Insurance Tuition Reimbursement Paid Time Off Immediate 401(k) eligibility Exceptional Corporate Support Equal Opportunity Employer We are an equal opportunity employer and prohibit discrimination/harassment without regard to race, color, religion, age, sex, national origin, disability status, genetics, protected veteran status, sexual orientation, gender identity or expression, or any other characteristic protected by federal, state or local laws.

Life-safety equipment checks (fire extinguishers, exit lighting, generator testing) happen on set schedules. Activities professionals plan and lead group programming β€” exercise classes, crafts, music sessions, outings β€” and also provide one-on-one engagement for residents who cannot participate in group settings. Social workers manage care conferences, discharge planning, family mediation, and community resource referrals.
