Apply today and grow with us! Occupational Therapist General Summary: Provide coordinated care to clients of all age groups. Plans, implements, and evaluates client care plans to restore or maintain patient well-being. Evaluates occupational roles and performance of clients with physical, psycho-social and/or cognitive impairments. Provides intervention to deficits and elicits responses. Patient Population: Has contact with patients of all ages in a clinical setting; understands and demonstrates appropriate behavior when interacting with patients in all populations. Essential Functions: Perform clinical assessments; identifies patient needs and appropriateness of client. Document, prepare/revises and completes a Plan of Care (POC), progress notes, phone orders, and other clinical record documentation on an ongoing basis. Apply concepts of infection control and universal precaution in coordination/performing client cares activities to protect both client and staff (OSHA). Assess and facilitate through direct therapeutic intervention client’s ROM, strength, coordination needs for and use of adaptive equipment and environmental modifications, functional mobility and safety issues as related to client’s ADL performance needs. Provides effective communication to clients, their families, team members, and other health care professional, and regular participation in Case Conference/Team Meetings. Facilitates the use of these skills by other team members to achieve desirable outcomes. Evaluate client occupational performance in the areas of self-care, work, leisure, and rest. Assess sensory-motor neuromuscular, psycho-social, and cognitive-perceptual components of occupational behavior. Monitors assigned cases to ensure compliance with requirements of third-party payers. Completes appropriate documentation in a timely manner to assure compliance with company policy. Demonstrates commitment and professional growth by participating in in-service programs and maintaining/improving competency. Meet mandatory continuing education requirements of the Agency/licensing board. This description is a general statement of required essential functions performed on a regular and continuous basis. It does not exclude other duties as assigned. Supervises: N/A Requirements Experience: Two (2) years’ experience as an OT in a clinical care setting. Preferred, home health experience. Skills: Therapy skills as defined as generally accepted standards of practice. Excellent interpersonal skills and ability to communicate effectively. Demonstrates proven decision making skills. If required to make visits, Hepatitis profile. Must read, write and comprehend English. Education: Bachelor of Science in Occupational Therapy. Licensure/Certification: Current driver’s license in good standing. It is the responsibility of the employee to renew their driver’s license before it expires in order to continue employment. Reliable transportation and auto liability insurance. Current State professional license as an OT in any/all applicable states. Current CPR. On-going employment as an OT always requires maintenance of a valid OT license in the state which the therapist practices in and a valid CPR certificate. It is the responsibility of the employee to renew these before they expire in order to continue employment. Physical Requirements: Prolonged standing/walking required. The ability to lift/transfer up to 50 lbs. Ability to meet deadlines and patient needs, make quick decisions and allocate resources under stressful conditions. Meet patient/family individualized psychosocial needs. Requires hand-eye coordination and manual dexterity. Environmental/Working Conditions: Works in patient’s home in various conditions. Possible exposure to blood borne pathogens, bodily fluids and infectious diseases. Some exposure to unpleasant weather; PRN emergency call. Ability to travel locally and work a flexible schedule.

Infection control protocols are more rigorous than in hotels or commercial cleaning, especially during flu season or respiratory illness outbreaks.

Maintenance staff handle a daily work order queue — everything from changing light bulbs and fixing call buttons to HVAC maintenance and plumbing repairs. Life-safety equipment checks (fire extinguishers, exit lighting, generator testing) happen on set schedules. Activities professionals plan and lead group programming — exercise classes, crafts, music sessions, outings — and also provide one-on-one engagement for residents who cannot participate in group settings.
