Job Description

Schedule: Full-time Monday–Friday occasional weekends (as needed) Location: Field-based Comp: $75.00 / point | $100,000 - $130,000 /year Who We Are Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model across Texas. Our ultimate mission is to dramatically improve patient access to home health care. The OT role is the front line of that mission: you restore function in the home, build safe routines, reduce avoidable decline, and help patients regain independence in ADLs/IADLs. We build technology to better support our field clinicians faster and more supported. Ultimately, our clinicians are the customers of our engineering team. Our charting product is designed to reduce documentation burden and keep visits moving, and our scheduling product (launching soon) will make it easier to coordinate visits, reduce last-minute changes, and protect clinician time. If you have strong clinical judgment and you like owning outcomes, this role gives you autonomy, meaningful patient impact, and a support team built to remove administrative friction. What you will do Complete in-home OT evaluations and deliver skilled OT visits. Build and execute a clear plan of care focused on ADLs/IADLs, safety/fall prevention, and functional independence. Recommend and train on adaptive equipment and home modifications as needed. Coordinate with nursing and the broader care team when condition or needs change. Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent. Requirements Must-haves Active Texas OT license (in good standing) Strong clinical judgment and comfort working independently in the field Comfort escalating changes in condition to RN/MD and documenting rationale clearly Strong communication and documentation Reliable follow-through on orders, frequencies, and patient goals Current driver’s license and comfort with daily travel across assigned territory Able to carry/transport basic therapy equipment as needed Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education) Nice-to-haves Prior home health experience Homecare Homebase (HCHB) experience (or similar home health EMR) Role: Occupational Therapist (OT) Deeper experience in high-signal OT populations (neuro, CVA, fall risk, dementia/caregiver training) Strong organization and time management

For housekeeping roles, the day follows a room-by-room schedule — cleaning resident rooms, sanitizing common areas, managing laundry, and responding to spill or accident cleanups as they happen. Infection control protocols are more rigorous than in hotels or commercial cleaning, especially during flu season or respiratory illness outbreaks.

Maintenance staff handle a daily work order queue — everything from changing light bulbs and fixing call buttons to HVAC maintenance and plumbing repairs. Life-safety equipment checks (fire extinguishers, exit lighting, generator testing) happen on set schedules. Activities professionals plan and lead group programming — exercise classes, crafts, music sessions, outings — and also provide one-on-one engagement for residents who cannot participate in group settings. Social workers manage care conferences, discharge planning, family mediation, and community resource referrals. Across all these roles, the common thread is that you become a familiar, trusted presence in residents' daily lives.
