Job Description

PharMerica is hiring a full-time Pharmacist in Houston, TX for our closed-door pharmacy team. Join us in a non-retail environment dedicated to serving long-term care and senior living clients.

What You'll Do

  • Evaluate physician medication orders for appropriateness, dosage, interactions, and administration routes
  • Provide clinical recommendations to physicians and healthcare staff regarding medication therapy optimization
  • Maintain up-to-date patient profiles including medical history, lab results, and current medications
  • Ensure laboratory tests for high-risk medications are ordered, reviewed, and communicated appropriately
  • Collaborate with physicians, nurses, and healthcare professionals to provide clinical support
  • Document communications and clinical interventions in patient medical records
  • Implement policies and procedures for safe medication handling, dispensing, and oversight
  • Ensure compliance with state and federal regulatory requirements
  • Report medication errors and adverse drug events to Pharmacy Manager
  • Educate clinical and healthcare agency staff on safe medication handling procedures

Requirements

  • Bachelor's Degree in Pharmacy; Pharm D preferred
  • One year experience in long-term care desired
  • Current and valid pharmacist license in state of practice
  • Knowledge of drug therapy guidelines, medication safety practices, and regulatory standards
  • Proficiency in electronic medical records and pharmacy information systems
  • Strong clinical judgment and effective written and verbal communication
