Job Description
PharMerica is hiring a full-time Pharmacist in Houston, TX for our closed-door pharmacy team. Join us in a non-retail environment dedicated to serving long-term care and senior living clients.
What You'll Do
- Evaluate physician medication orders for appropriateness, dosage, interactions, and administration routes
- Provide clinical recommendations to physicians and healthcare staff regarding medication therapy optimization
- Maintain up-to-date patient profiles including medical history, lab results, and current medications
- Ensure laboratory tests for high-risk medications are ordered, reviewed, and communicated appropriately
- Collaborate with physicians, nurses, and healthcare professionals to provide clinical support
- Document communications and clinical interventions in patient medical records
- Implement policies and procedures for safe medication handling, dispensing, and oversight
- Ensure compliance with state and federal regulatory requirements
- Report medication errors and adverse drug events to Pharmacy Manager
- Educate clinical and healthcare agency staff on safe medication handling procedures
Requirements
- Bachelor's Degree in Pharmacy; Pharm D preferred
- One year experience in long-term care desired
- Current and valid pharmacist license in state of practice
- Knowledge of drug therapy guidelines, medication safety practices, and regulatory standards
- Proficiency in electronic medical records and pharmacy information systems
- Strong clinical judgment and effective written and verbal communication
