The provider will manage these patients in a multidisciplinary value-based care model across multiple centers. The physician work assignments involve low-emerging risk complex patients to high complexity patients where the analysis of situations or data requires an in-depth evaluation of variable factors. The clinician will collaborate with a multidisciplinary care team and will serve as the leader of their respective care team within their Neighborhood Center. Essential responsibilities consist of but are not all inclusive: Responsibilities Cares for Suvida Healthcare’s patients and focuses on outpatient medicine, continuity of care, health maintenance, improving health outcomes, and disease prevention Manages patients in a multidisciplinary value-based care model Serves as the leader for their respective care team within the Neighborhood Center Collaborates effectively with Suvida Healthcare’s multidisciplinary care team Focuses on providing 5-Star care and strives to provide a high level of customer service Keeps a medical history and medical records Refers the patient to sp
