Description Advanced Houston Specialty Hospital is looking for a PRN Wound Care Nurse (LVN) . Starting rate: $35.00/hr This position is responsible for providing specialized care for patients with acute and chronic wounds, including pressure injuries, surgical wounds, diabetic ulcers, and other skin integrity issues. This role involves assessment, planning, implementation, and evaluation of wound care treatment plans in collaboration with interdisciplinary teams to promote optimal healing and patient outcomes. Essential Functions: Perform comprehensive wound assessments and document findings accurately. Develop individualized wound care plans based on evidence-based practices. Administer wound treatments including debridement, dressing changes, and negative pressure wound therapy. Monitor wound healing progress and adjust care plans as needed. Educate patients, families, and staff on wound prevention and management. Collaborate with physicians, physical therapists, dietitians, and other healthcare professionals. Maintain accurate and timely documentation in the electronic health record (EHR). Participate in quality improvement initiatives related to wound care. Ensure compliance with infection control policies and regulatory standards. Serve as a resource and mentor to nursing staff regarding wound care best practices. It is expected that the incumbent shall perform other duties as assigned within his/her capabilities as determined by management. Education and Experience Requirements: High school diploma or equivalent and successful completion of a vocational nursing program approved by the Texas Board of Nursing. Current, valid, and unencumbered Licensed Vocational Nurse (LVN) license in the state of Texas or compact state. Minimum of 2 years of clinical nursing experience required. At least 1 year of experience in wound care or related specialty preferred. Wound care certification preferred or must be obtained within 12 months of hire. Required certification and licensure as follows: Basic Life Support, Advanced Cardiovascular Life Support, Pediatric Advanced Life Support. Valid driver’s license with an acceptable driving record required, unless waived by management. Post Hire Requirements: Must successfully complete annual in-service training requirements and maintain appropriate facility clearance. Must complete facility-specific wound care training and competency validation. Participate in continuing education and professional development activities. Depending on the facility, you may also handle admissions paperwork, manage wound care, supervise medication techs, and respond to emergencies. The documentation load in senior living is significant — state surveys and compliance requirements mean thorough charting is non-negotiable. Many nurses in this setting say the most rewarding part is knowing their residents personally and being able to notice subtle changes that a rotating hospital nurse might miss.
