Altus Hospice is looking for a self-motivated and passionate Hospice RN Clinical Liaison to join our Team. The RN Clinical Liaison is primarily responsible for hospital and medical facility-based outreach to identify new referral sources and maintain ongoing relationships that will increase census and preference for hospice services. Registered Nurse Liaisons use their clinical knowledge, empathetic communication skills, and marketing abilities to initiate one-on-one dialogue with referral sources, families, and prospective patients to help explain and educate on hospice services. Promotes and positions Hospice's patients care service through regular visits in a manner that illustrates and reflects the Hospice's quality of care and unique capabilities. Works to build the brand of the hospice as the preferred hospice provider in our service area to achieve and exceed agency goals for growth and census management. Makes regular, planned calls to secure new referrals, build census, and meet established goals on new and existing, referring physicians; facilities; case managers discharge planners and social workers in hospitals and skilled nursing facilities; and other health care providers. May help Hospice in determining patient eligibility, based on their hospice knowledge and training. Conducts initial meeting with the patient and family giving an overview of hospice service and philosophy and obtains consents for care. Bachelor's degree in nursing or health care preferred. Holds current unencumbered license as a registered nurse. Minimum one (1) years of sales/marketing experience in healthcare. One (1) to two (2) years’ experience in hospice/palliative marketing strongly preferred. Skilled at establishing/maintaining working relationships with key staff, marketing contracts, and clinical staff to ensure thorough understanding of Hospice services. Proven ability to generate and maintain relationships with new and existing accounts including physicians, healthcare professionals, and community agencies. Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance. $90,000 - $105,000 per year (Average Pay Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.

What to Expect in This Role Day-to-Day

A typical shift starts with medication pass and resident assessments. You will review overnight notes from the previous shift, check vitals for residents on your watch list, and coordinate with CNAs on any changes in condition. Mid-shift usually involves documentation, family communication, and working with physicians on care plan updates — often by phone or telehealth rather than in-person rounds.

Depending on the facility, you may also handle admissions paperwork, manage wound care, supervise medication techs, and respond to emergencies. The documentation load in senior living is significant — state surveys and compliance requirements mean thorough charting is non-negotiable. Many nurses in this setting say the most rewarding part is knowing their residents personally and being able to notice subtle changes that a rotating hospital nurse might miss.
