Apply your expertise directly in a patient's most comfortable environment, delivering high-quality, personalized nursing care. Craft individualized care. Develop, implement, and dynamically adjust comprehensive care plans that promote patient independence and well-being. Collaborate and coordinate. Work seamlessly with a multidisciplinary team – physicians, therapists, and support staff – to ensure holistic care. Empower and educate. Serve as a vital resource for patients and their families, providing crucial teaching and emotional support. Mentor and guide. Oversee your caseload, leading and nurturing LPNs, LVNs, and Home Health Aides to foster a strong, cohesive care team. Innovate with technology. Utilize leading-edge tools and best practices to optimize care delivery and maintain precise documentation. Job Requirements Graduate of an accredited nursing program with an active RN license in the state served. Minimum of 1 year of nursing experience (Home Health experience is a significant plus!). Exceptional critical thinking, proven clinical expertise, strong case management abilities, and an unwavering commitment to patient-centered care. Current CPR/BLS certification. Valid driver’s license and reliable transportation. Depending on the facility, you may also handle admissions paperwork, manage wound care, supervise medication techs, and respond to emergencies. Many nurses in this setting say the most rewarding part is knowing their residents personally and being able to notice subtle changes that a rotating hospital nurse might miss.
