Care Manager – Private Duty Home Care Location: Houston, TX (Hybrid Field & Office-Based) Primary Service Area: Central Houston, River Oaks, West University, Memorial, and Tanglewood Schedule: Full-Time Compensation: $80,000–$100,000 base performance bonus opportunity up to $15,000 annually Our client, a nationally recognized and locally owned private duty home care agency, is seeking a highly polished, professional, and relationship-driven Care Manager to oversee client care delivery, caregiver oversight, and high-touch family relationships throughout Houston’s most prestigious neighborhoods. Key Responsibilities Client Care Leadership Conduct in-home consultations and develop customized care plans Serve as the primary point of contact for families, especially those with complex needs or high expectations Ensure care plans are executed with excellence, particularly during the first weeks of service Monitor client satisfaction and proactively address concerns Manage difficult family situations with empathy, professionalism, and strong communication Collaborate closely with scheduling and HR teams to appropriately match caregivers with clients Conduct quality assurance visits in client homes throughout the service area Caregiver Oversight & Team Leadership Build strong working relationships with caregivers while maintaining accountability and professionalism Lead and support Care Coordinators and care teams Conduct regular caregiver check-ins and supervisory support Reinforce professionalism, communication standards, and white-glove service expectations Assist with coaching, recognition, and caregiver development initiatives Representation & Community Presence Represent the organization professionally with clients, referral partners, and within the community Participate in community engagements, industry events, and professional networking opportunities as needed Help rei
