As a Care Transition Liaison (CTL-LPN) withPost Health Home Care and Hospice , you will play a key role in providing coordinated home health services for referred patients. You’ll collaborate with healthcare professionals, identify patient needs, and facilitate seamless transitions to home health care. Collaborate with hospitals, nursing homes, and home health personnel to ensure quality care. Must meet or exceed established monthly admission target(s) as provided by their supervisor and determined by the branch monthly budget. Coordinate patient referrals, evaluate eligibility for services, and assist in arranging home health care. Analyze patient needs and educate caregivers about home health options, Medicare/Medicaid guidelines, and insurance benefits. Develop relationships with healthcare providers to promote home health services and best practices. Participate in provider education and support hospital staff as a home health resource. Conduct trend analysis and identify opportunities for enhancing home health service outcomes. Contribute to Quality Assurance Performance Improvement (QAPI) initiatives. Graduate of an accredited school of nursing, current LPN license to practice in the state serviced Minimum of (2) years of healthcare experience, preferably in home health or sales/marketing. Strong communication, assessment, and decision-making skills. Ability to build relationships, educate, and promote services effectively. Mid-shift usually involves documentation, family communication, and working with physicians on care plan updates — often by phone or telehealth rather than in-person rounds.

Depending on the facility, you may also handle admissions paperwork, manage wound care, supervise medication techs, and respond to emergencies. The documentation load in senior living is significant — state surveys and compliance requirements mean thorough charting is non-negotiable. Many nurses in this setting say the most rewarding part is knowing their residents personally and being able to notice subtle changes that a rotating hospital nurse might miss.
