Job Description

Interim Healthcare, one of the nation’s leading providers of home care, is now seeking a dedicated and compassionate Admissions Nurse to service areas in and around the Houston area. The ideal candidate will provide Comprehensive Assessment and admission to patients in their homes, ensuring their health and well-being. Duties Conduct comprehensive patient assessments, admissions and develop individualized care plans. Provide education and support to patients and their families regarding health management Utilize aseptic techniques during procedures to ensure patient safety. Collaborate with interdisciplinary teams to coordinate patient care effectively. Document all patient interactions and care provided accurately in electronic health records. Requirements: Graduation from an accredited nursing school and a current, valid RN license registered in the state of Texas. Driver's License. Bilingual preferred. 1 year of home health nursing experience required. AI software must be used for every admission visit. Training and support will be provided to ensure you’re comfortable with the software. There are no fixed territories , this is a rover position , and flexibility is expected

Depending on the facility, you may also handle admissions paperwork, manage wound care, supervise medication techs, and respond to emergencies. The documentation load in senior living is significant — state surveys and compliance requirements mean thorough charting is non-negotiable. Many nurses in this setting say the most rewarding part is knowing their residents personally and being able to notice subtle changes that a rotating hospital nurse might miss.
