Work Overview We are looking for a Home Health Occupational Therapist Field Supervisor who will be responsible for supervising and providing support to home health Certified Occupational Therapy Assistants (COTA) while ensuring high quality care is delivered to patients in their home environments. The Field Supervisor will also build caseloads for the home health COTAs by conducting patient evaluations and ensure that patient goals are met in compliance with company standards and regulatory requirements. Key Responsibilities Serve as the primary point of contact for home health COTAs, offering guidance and support in clinical decision-making, patient care and documentation Conduct regular field visits to ensure quality care is being provided and offer mentorship to COTAs Monitor and evaluate COTAs' progress in treating their caseloads, providing constructive feedback if necessary Build and maintain caseloads for home health COTAs, ensuring an appropriate mix of patients to optimize productivity and treatment outcomes Collaborate with the team to adjust caseloads on therapist availability and patient needs Ensure that caseload management aligns with company policies, regulatory requirements and best practices Conduct at least 8 patient evaluations per week assessing patient needs and function to develop individualized treatment plans Benefits Benefits are offered to full time employees who work 30 hours or more a week.
