Description The Patient Care Manager is a registered professional nurse who ensures quality of patient care in the home. Primary function is for the overall administration of the clinical departments and monitoring of appropriate staffing and productivity. Coordinates care with the interdisciplinary group, patient, family and referring agency. Monitors and analyzes QAPI issues and directs clinical staff with regard to Hospice policies and procedures. May also be called Assistant Director of Nursing, ADON, or a similar title. Qualifications Education: Graduate from an accredited college / university with a degree in the discipline of the professional license held. Licensure: Active, unencumbered state license as a registered nurse, or a multi-state license issued by a Nurse Licensure Compact (NLC) member state. Current driver’s license. Experience: Minimum one year (two years for ACHC Accreditation) of hospice or home care experience as a registered nurse obtained within the last 36 months, experience in hospice preferred. Management experience preferred. Skills: Nursing skills as defined as generally accepted standards of practice. Excellent interpersonal skills and the ability to communicate effectively. Demonstrates proven decision-making skills. If required to make visits, proof of current CPR certification. Transportation: Reliable transportation and valid auto liability insurance.
