You'll: Complete comprehensive patient assessments and develop individualized care plans. Manage a designated patient caseload while coordinating care with physicians, therapists, and other healthcare professionals. Perform skilled nursing visits and evaluate patient progress. Educate patients and caregivers on medications, disease management, treatments, and preventive care. Identify changes in patient condition and adjust the plan of care as appropriate. Document care accurately and maintain compliance with all regulatory requirements. Advocate for patients while promoting independence, safety, and quality outcomes.
