Registered Nurse Assessment Coordinator (MDS) โ€“ We're hiring an RN to coordinate patient/resident care from admission through discharge. This role works under the Director of Nursing to manage assessments and care planning in a long-term care setting.

What You'll Do
  • Identify and monitor resident skilled services and care needs
  • Coordinate the Resident Assessment Instrument (RAI) process and complete accurate Minimum Data Sets (MDS)
  • Develop interdisciplinary Plans of Care with the healthcare team
  • Monitor resident status for changes in condition and progress toward care goals
  • Participate in daily Medicare Prospective Pay System Interdisciplinary Team Meetings
Requirements
  • Current, unencumbered RN license in state of practice (LVN licensure also considered)
  • Minimum 2 years direct hands-on nursing experience
  • Minimum 2 years long-term care experience
  • Preferably 1+ year RAI/MDS experience
  • ICD-10 coding proficiency and reliable transportation required

Depending on the facility, you may also handle admissions paperwork, manage wound care, supervise medication techs, and respond to emergencies.
