Social WorkerWhat You Must Have: Bachelors or Master’s degree in social work from a State accredited program Current State license required in accordance with State regulations Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance One year of experience as a Social Worker in a clinical care setting required, hospice experience preferred Full Time Employee Benefits Competitive Pay Medical, Dental & Vision insurance Paid Time Off Paid holidays 401k with up to 4% employer matching Tuition reimbursement Company car for qualifying individuals Mileage reimbursement What You Will Do: Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals. Provide individual patient/family counseling, crisis intervention, information and assistance with preparation of advance directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues. Facilitate community resources based on patient/family needs. Accept clinical assignments that are consistent with education and competence to meet the needs of the patients Perform on-call responsibilities and provide on-call service to patients/families as assigned Timely submission of all documentation Performance of other duties as required Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures We are looking for a compassionate, talented and experienced Social Worker that is experienced in customer service and eager to join an exciting organization.

Maintenance staff handle a daily work order queue — everything from changing light bulbs and fixing call buttons to HVAC maintenance and plumbing repairs. Life-safety equipment checks (fire extinguishers, exit lighting, generator testing) happen on set schedules. Activities professionals plan and lead group programming — exercise classes, crafts, music sessions, outings — and also provide one-on-one engagement for residents who cannot participate in group settings.
