This is a grant-funded position. Position Overview: The Clinical Health Services (CHS) License Vocational Nurse will support clinical services provided to patients by ensuring quality patient care through Immunizations and Family Planning Service programs. Under the clinical guidance/oversight of the CHS Advanced Practice Provider, the CHS Licensed Vocational Nurse will be responsible for performing clinical tasks in accordance with the federal, state, and local laws; rules, and regulations; and policies, procedures, and guidelines of Harris County Public Health (HCPH). This position supports the Harris County Public Health Department (HCPH), and reports to the Clinical Health Services (CHS) Program Manager. This position is apart of a mobile team and will service Harris County residents by providing comprehensive nursing services. Duties & Responsibilities: Administer age-appropriate immunizations and stay updated with all CDC vaccine schedule changes; assist the nursing team in providing CDC-recommended vaccinations . S chedule changes; assist the nursing team in providing CDC-recommended vaccinations. Assist the medical team in tasks such as (but not limited to) physical exams and medical procedures, administering and dispensing prescribed medication according to Pharmacy D regulations, patient education, and responding to emergency medical situations. Perform clinical tasks, including (but not limited to) measuring and documenting vital signs, collecting and preparing laboratory specimens, and conducting basic patient assessments. Receive, review, and prioritize all laboratory results. Ensure that all abnormal laboratory values are documented in the electronic medical record (EMR) system, and confirm that these values are reviewed by the medical provider for referrals or treatment as needed. Control patient flow through the facility during clinic visits, including discharging patients from the clinic following the medical provider's care plan and providing appropriate patient education. Participates in performing quality assurance checks on vaccines arrivals and preparing vaccines for transport. Manage medical inventory, including vaccines and oral medications, by ordering and ensuring the clinic is properly stocked. Participate in performing quality assurance checks on lab specimens and

A typical shift starts with medication pass and resident assessments. You will review overnight notes from the previous shift, check vitals for residents on your watch list, and coordinate with CNAs on any changes in condition. Mid-shift usually involves documentation, family communication, and working with physicians on care plan updates — often by phone or telehealth rather than in-person rounds.

Depending on the facility, you may also handle admissions paperwork, manage wound care, supervise medication techs, and respond to emergencies. The documentation load in senior living is significant — state surveys and compliance requirements mean thorough charting is non-negotiable. Many nurses in this setting say the most rewarding part is knowing their residents personally and being able to notice subtle changes that a rotating hospital nurse might miss.
