This role is ideal for a nurse with hands-on experience completing both skilled (Medicare) and long-term care MDS assessments, along with a strong understanding of wound-related coding and clinical documentation. Why Join Our Team Work Today, Get Paid Today – Daily Pay Available Competitive pay based on experience Medical, Dental & Vision Insurance 401(k) with Employer Contributions Paid Time Off Paid Holidays Company-Paid Life Insurance Tuition Reimbursement & CE Opportunities Supportive Leadership Team & Collaborative Environment Key Responsibilities Complete and submit accurate, timely MDS assessments (MDS 3.0) for both skilled (Medicare) and long-term care residents Set, track, and manage Assessment Reference Dates (ARDs) Ensure documentation supports clinical conditions, including wounds and complex care needs Conduct and complete Care Area Assessments (CAAs) Collaborate with the interdisciplinary team on care planning and resident outcomes Review clinical documentation for accuracy, compliance, and reimbursement optimization Maintain up-to-date knowledge of Medicare/Medicaid guidelines and regulatory requirements Support survey readiness and participate in audits as needed Qualifications (Required for Consideration) Active RN or LVN license in the state of Texas (required) Proven experience completing MDS assessments in a skilled nursing or long-term care setting (required) Experience completing skilled (Medicare) MDS assessments (required) Experience with long-term care residents and wound-related MDS coding/documentation (required) Strong knowledge of: MDS 3.0 process RAI (Resident Assessment Instrument) Manual Care Area Assessments (CAAs) Medicare/Medicaid reimbursement guidelines Strong attention to detail and organizational skills Preferred Experience 1–2 years of experience in a skilled nursing facility (SNF/LTC) Experience with wound care, rehab, or case management Familiarity with PointClickCare (PCC) or similar EMR systems What We’re Looking For A detail-driven nurse who understands the financial and clinical impact of MDS accuracy Someone who can work independently while collaborating with the care team A professional who takes ownership of timelines, compliance, and quality outcomes Job Disclaimer This job description is not intended to be all-inclusive.
