Position Overview Collage Home & Community is seeking an experienced Registered Nurse (RN) to serve as a Nurse Supervisor supporting our Texas Licensed Home Health Services (LHHS) operations. This role will initially begin as PRN and is expected to ramp quickly to a full-time position as new clients are onboarded and service volume grows. This is an excellent opportunity for a clinically strong RN who wants to help build and scale a high-quality, compliance-driven home health program focused on medically complex patients. Role Structure (Important) Initial Status: PRN (flexible scheduling) Expected Transition: Full-time as patient census increases Ramp Trigger: Growth in skilled nursing and CNA-supervised cases Supervision Model: RN-led clinical oversight under Texas LHHS regulations This position is intentionally structured to allow the right candidate to grow into a long- term leadership role as the program expands.
