Description Pediatric Transitional Nurse – Registered Nurse (RN) / Licensed Vocational Nurse (LVN) Houston, TX | Bilingual Preferred (Not Required) Pay Range: LVN: $30–$35/hour RN: $35–$42/hour Make a Difference Beyond One Shift Angels of Care Pediatric Home Health is hiring Registered Nurses (RN) and Licensed Vocational Nurses (LPN) for our Pediatric Transitional Nurse Team to support medically fragile children and their families during critical transitions to home care. This role is ideal for nurses who are confident working with high-acuity patients and skilled in trach, vent, G-tube, and emergency management . Nurses in this position play a critical role supporting families, training caregivers, and ensuring safe continuity of care in the home environment. Meaningful one‑on‑one pediatric care Flexible scheduling with on‑call support Schedule set 1 week in advance Clinically led, nurse‑founded organization Supportive team culture with a strong mission Opportunity to train and mentor other nurses Comprehensive benefits package Mileage Reimbursement What You’ll Do Provide skilled pediatric nursing 1:1 care in patients’ homes Perform patient assessments and ongoing evaluations -Administer medications, treatments, and therapies Support hospital-to-home transitions for high-acuity patients Educate and support parents, families, and caregivers Assist with discharge support and care coordination Train and support permanent field nurses during transitions Provide first-shift and interim coverage as needed Collaborate with Clinical Care Managers and interdisciplinary teams Respond appropriately in emergency situations and changes in patient condition Qualifications Required: Active Texas Registered Nurse or Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) license Current CPR certification Ability to lift and assist pediatric patients as needed Experience with Trach care, Ventilator management, and TPN High-acuity pediatric patients (preferred) Emergency response management Preferred: Pediatric home health experience High-acuity pediatric patients (preferred) Schedule & Travel Full-time position, with guaranteed hours Flexible scheduling based on patient needs and Nurse availability 5 working days per week , minimum 1 weekend day On call and Staffing coverage responsibilities as needed Local travel within approximately 30–40 miles Why Angels of Care: We have a full benefits package, bulleted out below. Depending on the facility, you may also handle admissions paperwork, manage wound care, supervise medication techs, and respond to emergencies. The documentation load in senior living is significant — state surveys and compliance requirements mean thorough charting is non-negotiable. Many nurses in this setting say the most rewarding part is knowing their residents personally and being able to notice subtle changes that a rotating hospital nurse might miss.
