Job Description
Join Humana's Primary Care Organization as a Primary Care Physician leading a multi-disciplinary care team. Spend 100% of your time delivering direct patient care in a value-based, team-based environment focused on senior wellness.
What You'll Do
- Evaluate and treat center patients according to standards of care and quality measures
- Coordinate patient services including specialty referrals, hospital coordination, and home health care
- Participate in daily care team huddles as key source of medical expertise
- Complete medical record documentation and work with quality-based coders on coding optimization
- Participate in on-call program and potential growth opportunities for new services
Requirements
- Current, unrestricted medical license or willingness to obtain in state of practice
- Graduate of accredited MD or DO program from accredited university
- Excellent verbal and written communication skills and strong interpersonal abilities
- Fully engaged in integrated team-based care model and value-based care settings
- Basic computer skills including email and EMR; patient-facing role with TB screening program participation

Maintenance staff handle a daily work order queue — everything from changing light bulbs and fixing call buttons to HVAC maintenance and plumbing repairs. Life-safety equipment checks (fire extinguishers, exit lighting, generator testing) happen on set schedules. Activities professionals plan and lead group programming — exercise classes, crafts, music sessions, outings — and also provide one-on-one engagement for residents who cannot participate in group settings. Social workers manage care conferences, discharge planning, family mediation, and community resource referrals.
