Use professional nursing knowledge, critical thinking skills and dialysis experience to serve as a resource to staff, Physicians and Physician�s Assistants. Minimum Qualifications: Education and Experience: Graduate of an accredited nursing program required; Associates Degree in Nursing (ADN), Bachelor of Science in Nurse (BSN) and/or Master of Nursing (MSN) Current state licensure as a Registered Nurse Current CPR certification (AHA BLS) 18 months of clinical nursing experience to patients on maintenance dialysis within the last 24 months. Required Skills and Competencies: To maintain on-call status on an intermittent or regularly scheduled basis. Ability to satisfactorily communicate in English including reading, writing, speaking, and understanding Computer proficiency Essential Duties and Responsibilities: In conjunction with the Vice President, ensures dialysis unit operates in compliance with all ESRD, HCFA, federal, AAMI, OSHA and company policies and procedures Responsible for training and development of Skilled Nursing Facility staff while providing clinical oversight to non-licensed support team delegating tasks as appropriate Knows and follows policies and procedures under CMS ESRD and state regulations and Rendevor Dialysis, monitors staff compliance of same Monitors and assures patients receive adequate medical attention during dialysis treatments and pertaining to dialysis care while a patient of the Skilled Nursing Facility in the dialysis unit Conducts comprehensive assessments that include the medical, behavioral, pharmaceutical, and social needs of the patient, identify gaps in care and barriers to good health Based on this assessment, and in conjunction with the patient care team, create and implement a care plan that will address identified needs, remove barriers to care, and improve the health of the patient Coordinates care by serving as the advocate and resource for the patient, their family, and their provider(s) Facilitates continuity of care, spanning settings such as the home, hospital, skilled nursing facility, and acute care facility Manages patients during periods of transitions of care minimize avoidable readmissions Assesses the patient's knowledge of their renal condition and provide education and self-manage

What to Expect in This Role Day-to-Day

A typical shift starts with medication pass and resident assessments. You will review overnight notes from the previous shift, check vitals for residents on your watch list, and coordinate with CNAs on any changes in condition. Mid-shift usually involves documentation, family communication, and working with physicians on care plan updates — often by phone or telehealth rather than in-person rounds.

Depending on the facility, you may also handle admissions paperwork, manage wound care, supervise medication techs, and respond to emergencies. The documentation load in senior living is significant — state surveys and compliance requirements mean thorough charting is non-negotiable. Many nurses in this setting say the most rewarding part is knowing their residents personally and being able to notice subtle changes that a rotating hospital nurse might miss.
